How to Apply

To apply, submit a completed T.E.A.C.H. application with:

Center Provider or Group Home Provider

  1. Proof of income and documentation of number of hours worked per week

    A) A current pay stub that indicates: hourly wage, number of hours worked per week and length of pay period.

    OR

    B) A letter from the center director (on company letterhead) stating hourly wage and number of hours worked per week.

     

  2. Documentation of acceptance or denial of PELL Grant.
  3. Documentation of articulation of 55 semester/83 term credit hours toward Bachelor degree in Child Development or Early Childhood Education.
  4. Appropriate signatures on application for sponsorship.

Family / Group home owners

  1. Verification of income includes completion of a monthly income worksheet (provided with application). Both sides must be completed.
  2. Copies of DHS reimbursement statement and Food Care Program are also required (if currently participating).
  3. Documentation of acceptance or denial of PELL Grant.
  4. Documentation of articulation of 55 semester/83 term credit hours toward Bachelor degree.
